Exciting opportunity! We’re seeking an experienced RGN with expertise in Neurological Rehabilitation to join a dynamic Early Supported Discharge Service.

Responsibilities:

• Ensure stroke survivors have access to a top-notch, stroke-specific ESD service.
• Facilitate smooth transfers of care from hospital to home or Community Stroke rehab.
• Enable intense rehab in-home or community settings, matching acute inpatient care.
• Collaborate to identify suitable patients for the service.
• Assist stroke survivors in achieving rehab goals for maximum recovery.
• Involve survivors, carers, and families in the rehabilitation process.
• Provide specialized stroke rehab in a multi-disciplinary team.

Requirements:

• RGN with Neurological Rehabilitation expertise.
• Valid driving license and own transport.

💰 Pay Rate:
